you should have disabled your SP lock before you disabled your Network Lock. to fix this, in your case, do the following (no "*2767*3855# - Full e2p reset" required because you have already done one):

2. *7465625*77*00000000*00000000# - Relock SP lock & reset to 8 zeroes
3. #7465625*77*00000000# - Disable your SP lock
4. #7465625*638*00000000# - Disable Network lock
5. phone should be ok and unlocked
